Registration Fee

 

1) ICORD Regular: JPY 20,000 (USD 250)

Include with Participation fee of all main and poster session / Program/ Welcome Reception (Feb. 4th) / Banquet (Feb. 5th) /Coffee/Luncheon Seminar　etc.
Please select the number of "working group" on the registration form.

2) Caregiver: JPY 1,000 (USD 13)

Only care giver can register to this type. Not include with program agenda, Welcome Reception and Banquet fees. Please note the patients’ name on the comment　column. Patient also needs to register as ICORD regular.

3) Welcome Reception fee: JPY 5,000 (USD 65)

You can attend the Welcome Reception at February 4th. The venue is Komaba Research Campus, The University of Tokyo.　Object person is family/ accompanying guest(s) and Type(2).
You can select both type: (3) and (4).

4) Banquet fee: JPY 5,000 (USD 65)

You can attend the Banquet at February 5th. The venue is Komaba Research Campus, The University of Tokyo. Object person is family/ accompanying guest(s) and Type (2).
You can select both type: (3) and (4).

All types are not included with staying cost / travel cost / breakfast / dinner.

CANCELLATION BY DELEGATE:

All cancellations must be made in writing to icord2012contact@prip-tokyo.jp.

 

For cancellations received and confirmed before the end of November 2011

  - a 90% refund.

For cancellations received and confirmed during December 2011

  - a 50% refund

For cancellations received and confirmed after January 1st 2012

  - no refund will be given but a substitute delegate may be nominated.

 

There are no reasons for delegate cancellation which make the above schedule of cancellations inapplicable. Refunds will be made within 30 days of receipt of cancellation notification.
